About the Practice

We are delighted to welcome you to Homewell Practice, where we aim to provide a high standard of health to all our patients. If you have not yet registered with the practice, please contact us to arrange registration or complete the registration form on our website. You will need to visit the surgery with identification in order to complete registration.

The doctors are mostly partners at the practice and we are happy for you to consult any members of the practice, but it is to your advantage to have continuity of care from one doctor. You may express a preference in writing to receive services from a particular doctor/health care professional either generally or in relation to any conditions. We endeavor to comply with reasonable requests but reserve the right to not do so if the preferred person has reasonable grounds.

Statement of Purpose

Our aims are to prevent ill health, improve the wellbeing of our patients, work with our local commissioning group and other integrated health teams to provide services that improve local health outcomes and reduce admissions to secondary care.

Gold Standards Framework (GSF)

We at Homewell Practice adhere to the Gold Standards Framework (GSF). The National Gold Standards Framework Centre (GSF) provides the highest possible standard of care for all patients who may be in the last years of life.

Our three steps are: identifying the patient, assessing the patient and plan.

GSF is all about quality of life. We try to ensure that you and your family are helped to be as physically well supported, to feel safe, informed and reassured of support.

We meet monthly with our palliative care lead, local Hospice, District Nurses and other professionals. We work as a team to help provide the highest standard of care possible for patients and their families by discussing each patient and recognising the care and support you will, or are now needing.
